Referrals

PATIENTS WHO WERE TAKING ANTICOAGULANTS PRIOR TO ADMISSION

  • Determine who regularly manages the patient’s anticoagulant  therapy, and where the patient typically goes for blood draw
  • Call the provider currently managing anticoagulant therapy to re-refer the patient
  • Schedule an appointment:
    • In 1-3 days if patient and/or therapy are not stable
    • In 3-7 days if patient and/or therapy is stable
  • Forward or fax the inpatient anticoagulation flow sheet, discharge med list and discharge summary to the provider currently managing anticoagulant therapy

PATIENTS NEW TO ANTICOAGULANT THERAPY

  • Select an appropriate strategy for management. Options include:
    • UWMC Montlake Anticoagulation Clinic (for referrals from UWMC Montlake-based providers, and providers at Stadium, Hall Health and Roosevelt clinics)
    • UWMC Northwest Anticoagulation Clinic (for referrals from UWMC Northwest-based providers)
    • SCCA Anticoagulation Clinic (for referrals from SCCA-based providers)
    • HMC Anticoagulation Clinic (for referrals from HMC-based providers)
    • For patients with a UWPN (Neighborhood Clinic) PCP and no speciality care at UWMC/HMC/SCCA – refer to PCP
    • For patients with a non-UWMedicine PCP and no speciality care at UWMC/HMC/SCCA – refer to PCP
  • If UWMC, SCCA or HMC Anticoagulation Clinic is selected:
    • If referring from an inpatient setting or from within SCCA, forward or fax  the completed Referral Treatment Plan Form (attached below) (as well as inpatient anticoagulation flow sheet, discharge medication list and discharge summary for inpatients)
    • If referring from an outpatient clinic using EPIC, enter an EPIC referral to UWMC-ML, UWMC-NW, or HMC ACC
    • Anticoagulation Clinic will contact the patient to set up the first visit
  • For all other management options:
    • Call the follow-up provider or anticoagulation clinic to refer the patient
    • Schedule an appointment:
      • in 1-3 days if therapy is not stable
      • in 3-7 days if therapy is stable
    • Fax the inpatient Anticoagulation Flow Sheet, discharge medication list and discharge summary  to the follow-up provider or anticoagulation clinic
