Health Requirements

Employee Health is your one stop shop for most of your pre-orientation health care tests. There are several Employee Health locations, please be sure to go through UWMC-Montlake Employee Health location.

Plan on completing the following health requirements prior to 7/2.

Please gather these documents and preferably send them to us prior to arriving for the immunization review.

Vaccine Requirements at the UW

At the UW, you are categorized as Professional Staff (not a trainee) which includes many benefits and requirements. All new staff are required to provide proof of vaccination for more than just Covid-19. You may be able to send your records via email but check with Employee Health at emphlth@uw.edu or (206) 685-1026.

Covid Specific Vaccine Information

The University of Washington requires all faculty, staff, and students to be fully vaccinated for Covid-19. Details on the UW’s Covid Policies are here: https://www.washington.edu/coronavirus/vaccination-requirement/

Tuberculosis

  • TB Risk Assessment
  • Tuberculosis Symptom Survey
    • If you have had a test within the last year, you only need one, 30 days before 7/2
    • If you need two, they need to be taken 1 week apart, the last one 30 days before 7/2
  • If history of Positive TB – please bring documentation of TB test, copy of Chest X-ray. If prophylaxis (INH, etc.) taken documentation of this too.
  • If history of Negative TB — Documentation of TB testing completed in the last month or we will draw blood for TB (Quantiferon Gold)

Other Immunizations

  • Current Tdap/TD Vaccine with in past 10 years. If providing TD we will also need documentation of a previous Tdap vaccine.
  • Record of 2 MMR vaccines or Positive titers for Measles (Rubeola), Mumps and Measles
  • Record of 2 Varicella vaccine or Positive titer for Varicella
  • Hepatitis B vaccine series and Positive Hepatitis B Surface antibody titer

You will also need

  • Respirator fit test – in person at Employee Health
  • do NOT eat or drink for 15 min. prior to the test
  • Required for all tracks, except for residents who exclusively work at SCH.
