Checklist – Human Resources

The Dept of Psychiatry & Behavioral Science’s Human Resources team will be sending you requests and information about onboarding once your Hire Profile is complete. These are some of the processes that have to take place to get you onboarded.

• Background checks (WA state and national) including a Conviction/Criminal History

• Employee Health – required – acomplete these requirements prior to Orientation. Please plan in advance if you can. Check in at UWMC Employee Health to review your immunizations and to administer the following tests. You can just walk in to the clinic at BB306, no appointment necessary.

  • Fit Test (required)

  • TB test (this may not be needed this year, we are awaiting more information)

• Identification

  • Husky Card

  • I-9 and W-2 forms

  • UWNetID

  • EID (Employee Identification)

  • UWMC ID Badge

  • HMC ID Badge

  • UWMC AMC user ID

• EMR / EPIC Training

• Photo and bio

• Training

  • HIPAA

  • Asbestos

  • Child Abuse

  • Violence Prevention
