The Jumpstart Program is a quality improvement program using one-page “Jumpstart” guides to increase the occurrence and quality of goals-of-care discussions between clinicians and patients.

We currently are implementing two different Jumpstart studies — one for patients in the hospital, and another for patients in the outpatient setting. See below for FAQs specific to the setting where you received the Jumpstart.
