Ordering Controlled Substances

License Requirements

  • MD’s, DO’s, DDS’s, DVM’s and certain midlevel practitioners (as defined by DEA), who possess a clinical DEA registration, may use that registration to obtain controlled substances for research, as long as the schedules listed under their clinical registration include the substances with which they wish to order. If you have a clinical DEA registration a separate state registration is not required. A copy of the current registration certificate must be provided to UW Medicine’s Drug Services prior to ordering.
  • Principal Investigators and research staff who do not have a clinical DEA registration will have to obtain and provide evidence of current registrations with the DEA and the Department of Health (DOH) in order to obtain controlled substances from Drug Services Division.

Registrants are responsible for providing Drug Services Division with copies of their current registration certificate(s) annually if they wish to purchase controlled substances. Copies of registrations may be faxed directly to us at 206-598-3808. Drug Services will send registrants one email reminder when their registration documentation needs updating.
In addition, under the state and federal registration systems, all Schedule II controlled substance orders (e.g., Pentobarbital, Fentanyl) must also be accompanied by a completed DEA Schedule II Order blank (Form 222). This form is supplied to you by the DEA. Drug Services Division is not registered to provide or handle C-I controlled substances.

Ordering

For orders of DEA Schedule II, III, IV, or V controlled substances, submit a registrant signed and dated Drug Services Controlled Substance Order form (found at the top of this page) via fax, email attachment, mail, or hand delivery. Each controlled substance ordered requires a separate form. We cannot accept email or telephone orders for controlled substances. Orders submitted as email attachment are acceptable.

It is recommended to order no more than a 30-day supply.

Picking Up an Order

Controlled substances should be picked up from Drug Services Division within 14 days of ordering.

  • A valid state driver’s license or government-issued ID (e.g., passport) is required for pick up by the registrant or its designee.
  • The registrant or its designee will be required to sign for the controlled substance order.
  • For Schedule II controlled substances, a DEA 222 form with name printed, signed, and dated by the registrant is required for pick-up.

Controlled Substance Storage and Use

The registrant is responsible for the proper storage and record keeping.

  • Controlled substances must be stored in a locked cabinet with limited access.
  • A perpetual inventory must be maintained.
  • Inventory forms must conform to DEA and State regulations.  Drug Services can provide sample forms that have been approved by the Board of Pharmacy.
  • All controlled substance records must be maintained for a minimum of 3 years.
  • Any unexplained/unaccounted drug losses or discrepancies discovered require notification to Drug Services, the Board of Pharmacy, and the DEA.

Disposal of Expired or Wasted Controlled Substances

Drug Services is not authorized to accept controlled substances for disposal.

  • Expired or waste (undesired) drugs must be kept secure in a locked cabinet and properly labeled for content in a separate container until disposal.
  • A perpetual log noting the waste containers’ contents must be completed and signed with each entry of waste.
  • Drug Services will provide regular collection dates with a licensed reverse distributor, to whom the waste material can be transferred for disposal.
  • The reverse distributor will provide a DEA 222 form for the transfer of CII waste.
  • The records of the transfer should be filed with the invoice records.
  • Drug material shall not be disposed of via any garbage or sewer disposal mechanisms.
