This is the survey for people who are experiencing worsening confusion or memory loss, or have a diagnosis of mild cognitive impairment or early stage dementia.
Changes in thinking and memory can impact daily activities and social interactions with other people, especially family and close friends. Healthcare providers need to better understand the experiences of people who are experiencing thinking and memory changes AND their family / close friends in order to learn about how to better support everyone involved. We appreciate you sharing your experiences with us!

About the Staying Connected through Communication Survey
Do you qualify for this survey study?
We are looking for survey participants who:
- Are age 18 years or older
- Live in the United States
- Do NOT live in a nursing home or similar medical facility (it is ok if you live in a
retirement home that does not provide medical care) - Speak English comfortably enough to complete a questionnaire and participate in an interview
- Have experienced worsening or more frequent confusion or memory loss for at least three
months - Have been diagnosed with or have concerns about possible mild cognitive impairment and/or dementia
- Do NOT have a diagnosis that could impact your speech, communication, or thinking other than dementia. For example, you do NOT have primary progressive aphasia, Parkinson’s disease, multiple sclerosis, stroke, or traumatic brain injury
