• Missouri: SNF 30 minutes south of Joplin
• North Dakota: SNF in NW ND near Dakota Park
• Louisiana: SNF 45 minutes northeast of Alexandria
• Alabama: SNF an hour northwest of Dothan
• Kansas: SNF in the Coffeyville/Independence area (CCCs preferred)

Adult/Medical – CCCs required
• Massachusetts: SNF in the Peabody area
