CF Opportunity at the West Palm Beach VA Medical Center

The West Palm Beach VA Medical Center is a dynamic hospital comprised of 180 hospital beds (Intensive Care, Acute Medicine, Blind Rehabilitation and Psychiatric Health) as well as 120 Community Living Center (CLC) beds and a dedicated Hospice Unit.  The WPB VAMC serves as a Polytrauma Clinic Support Team (Level III). In addition, we provide telemedicine and homecare services to veterans in the community. Veterans also participate in treatment and support groups facilitated by the speech-language pathologists focusing on Parkinson’s Disease, Post-CVA Communication Skills, and Laryngectomy.

The Clinical Fellow will engage in a professionally challenging year gaining experience in evaluation and evidence based treatment of adults with dysphagia, cognitive-communication impairments, aphasia, dysarthria and voice disorders. The candidate will also develop skills in fluoroscopic and endoscopic swallow evaluations, endoscopic voice evaluations, and post-laryngectomy voice restoration. In addition, the Clinical Fellow will receive training in assistive technology (AAC) for augmenting communication and cognition. We also work closely with the OEF/OIF/OND population, addressing their speech, language, and cognitive concerns. Veterans are seen along the continuum of care, including ER admission, intensive/acute care, short-term rehabilitation, long-term care, outpatient and telemedicine rehabilitation. Our Speech-Language Pathology section maintains close interaction with a variety of specialty services, including Audiology, Otolaryngology, Radiology, Neurology, Psychology, Social Work, Pulmonary Medicine, Neuropsychology, Physical Medicine & Rehabilitation, Nutrition & Food Service, Blind Rehabilitation, and Prosthetics & Sensory Aid Service. The Speech-Language Pathologists coordinate the Comprehensive ALS Interdisciplinary Team at our facility, which will provide a rich clinical experience.

 

GENERAL INFORMATION:

CF Position Available in the Washington DC VA Medical Center

The clinical fellow will gain experience in evaluation and treatment of adults with traumatic brain injury, cognitive communication impairments, aphasia, dysarthria, dysphagia, and dysphonia. Clinical fellows will also receive training in performing modified barium swallow and FEES studies, assessing patients for use of assistive technology, transgender voice therapy, and providing pre- and post- laryngectomy education and voice restoration. The CF will manage his/her own caseload and see patients along the continuum of care from intensive care units, outpatient care, sub-acute rehab, long-term care, and hospice/palliative care. He or she will gain experience in conducting both individual and group therapy sessions. The clinical fellow will also interact with a variety of specialty services including neurology, rehab medicine, ENT, geriatrics, and primary care and have an opportunity to participate in interdisciplinary team meetings.

CF Position in the New Mexico VA Health Care System

The New Mexico VA Health Care System is pleased to announce a speech-language pathology clinical fellowship (CF) position for the 2020-21 year. The Clinical Fellowship provides a well-rounded experience in the evaluation and treatment of adults in all areas of medical speech pathology including dysphagia, speech and language disorders, cognitive disorders, voice disorders, and tracheostomy. Specialty areas in head & neck cancer, neurodegenerative diseases, TBI, trach/vent, AAC, and transgender voice are also offered.  Our department serves inpatients, outpatients (in-clinic and telehealth), and rehabilitation residents of the Community Living Center.

Over the course of this year long rotation,  the Clinical Fellow can expect to have a combination of inpatient and outpatient experience to include training in swallow evaluations (bedside, fluoroscopic, and endoscopic), endoscopic voice evaluations (flexible and rigid), post laryngectomy voice restoration, language and cognitive therapy, participation in comprehensive rehab teams for ALS, Parkinson’s, and Spinal Cord Injury (SCI), as well as assistive technology for augmentative communication. The CF will work with a variety of specialty services and teams via interdisciplinary rounds and collaboration, including: Otolaryngology, General Medicine Hospitalist teams, Nursing, Neurology, Neuropsychology, Rehabilitation Medicine, Nutrition, SCI, and Primary Care.

Located in the high desert of northern New Mexico, Albuquerque experiences four seasons a year which allows for the enjoyment of one of the most diverse geographical regions in the country year-round. A CFY position in speech-language pathology is available through the Geriatric Research Education and Clinical Center (GRECC) at the William S. Middleton Veterans Affairs Hospital in Madison, Wisconsin. This will be a blended fellowship training experience incorporating training through the Speech Pathology section and the GRECC. The Speech Pathology Section serves inpatients, outpatients, and residents of a Community Living Center. CF will interact professionally with specialty services including Neurology, ENT, Med/Surg, Nutrition, Nursing, and other rehabilitation therapies. All CFs will gain experience and provide services in all areas of medical speech pathology, including evaluation/treatment of individuals with dysphagia (videofluoroscopy), speech and language disorders, cognitive disorders, and voice disorders. They will also gain specialized experience in cognitive rehabilitation for veterans with mTBI, neurogenic communication disorders, endoscopic procedures (FEES and videostroboscopy), head and neck cancer, and laryngectomy care. CF will also participate in several weekly Geriatric based interdisciplinary clinics. Trainees will be integrated directly into these interdisciplinary geriatrics teams and through administration of standardized screening tools, will identify those Veterans who would benefit from additional dysphagia, speech/language, or cognitive intervention from the Speech Pathology service. We are seeking candidates with a strong interest in medical speech pathology, geriatrics, and dysphagia research, as well as a desire to grow professionally in a fast-paced, learning-oriented medical setting.

Examples of Duties:
  • Administers standardized and/or supplemental assessments (e.g. hearing screenings, etc.) for the purpose of identifying students with communication disorders.
  • Assesses students’ communication skills (e.g. articulation, fluency, voice, expressive and receptive language, etc.) for the purpose of identifying communication disorders, determining program eligibility and developing recommendations for treatment.
  • Collaborates with a variety of groups and/or individuals (e.g. parents, teachers, physicians, administration, maintenance personnel, team members, other professionals, etc.) for the purpose of communicating information, resolving issues and providing services in conformance with established guidelines.
  • Consults with teachers, parents, other personnel and/or outside professionals for the purpose of providing requested information, developing plans for services, making recommendations and/or coordinating speech/language therapy services with those of other disciplines.
  • Coordinates meetings and processes for eligible students (e.g. testing/screening, IEPs, parent conferences, pre-referral staffing, GQP meetings, etc.) for the purpose of presenting evaluation results, developing treatment plans, and/or providing training to parents/students/staff.
  • Designs therapeutic strategies for the purpose of modifying communicative behavior and/or identifying appropriate assistive devices.
  • Develops treatment plans, interventions and/or educational materials for the purpose of minimizing the adverse impact of communication disorders in conformance with state and federal regulations.
  • Directs the work of assigned Communication Therapy Aides for the purpose of providing guidance and ensuring that program objectives are achieved.
  • Instructs Communication Therapy Aides and other staff for the purpose of providing information on communicative disorders, use of assistive devices and feeding techniques and/or implementing prescribed treatment plans.
  • Instructs eligible students in the use of appropriate communication technologies (e.g. hearing aids, FM systems, augmentative communication devices, etc.) for the purpose of minimizing the adverse educational impact of communication disorders as per state and federal regulations.

Over the course of the rotation, the Clinical Fellow can expect to have a combination of inpatient and outpatient experience and includes training in performing swallow evaluations (bedside, fluoroscopic, and endoscopic), endoscopic voice evaluations, post laryngectomy voice restoration, language and cognitive evaluations and therapy, as well as assistive technology for augmenting communication and cognition.

CF Positions Open in Minneapolis VA Health Care System

The Speech-Language Pathology Section at MVAHCS is pleased to announce two (2) Clinical Fellowship positions available for Fiscal Year 2021, with desired start in Summer 2020.

Clinical Fellows participate in a truly interdisciplinary team approach to evaluation and treatment of veterans and service members served within the medical center. Veterans served cross the age span from young adult
through older adult populations. CFs evaluate and treat communication, cognitive and swallowing disorders related to stroke, head and neck cancer, TBI & Polytrauma and a wide variety of other medical diagnoses. They may provide aural rehabilitation and Transgender communication services as well. CFs receive training in instrumental assessment and treatment procedures including modified barium swallows, FEES, and stroboscopy.

Many therapies may be delivered via a range of Clinical Video Telehealth options. CFs are active participants in the care of patients in medical and surgical wards, rehabilitation units (acute, subacute), outpatient clinics, and intensive care units. They participate in interdisciplinary conferences and rounds, family conferences, co-treatments with other service providers, group therapy sessions, community outings and local and national outcomes data collection (e.g. FIM, NOMS and other outcome measures). There may be opportunities to participate in research as well. CFs are strongly encouraged to take advantage of the many continuing education experiences offered within the hospital and community. The Minneapolis VA Health Care System is a specialty site for Assistive Technology, a TBI Center of Excellence, and is one of five recognized Polytrauma Centers in the VA. MVAHCS offers CARF-accredited programs in inpatient rehabilitation for amputation, brain injury, stroke, and
spinal cord injury; residential transitional rehabilitation for brain injury, outpatient brain injury and spinal cord injury, assistive technology, and chronic pain.

The setting includes experiences in acute and critical care, outpatient clinic, and the CLC, which provides short-term rehab, respite, hospice/palliative care,  and long-term care.  Outcomes/Evidence based practice for treatment across communicative, cognitive, and swallowing disorders is emphasized.
