- HQSC Online Library: Members can access useful tools from our shared Google Drive including the UW Internal Medicine QI Handbook, a set of QI Commandments, a project charter template that doubles as a refresher to the IHI Model for Improvement, a development checklist that offers step-by-step guidance from project inception to report out, and statistical process control (SPC) chart Excel worksheets. SPC chart files are provided courtesy of the VA National Center for Patient Safety.
- Institute for Healthcare Improvement (IHI) Open School: Learn the fundamentals of patient safety, quality improvement, leadership, and patient-centered high-value care through modular online self-study, with a particular focus on the Model for Improvement framework. HQSC members are required to complete the IHI Open School Basic Certificate in Quality & Safety in order to qualify for the UW GME Graduate Quality & Safety Certificate.
- Agency for Healthcare Research and Quality (AHRQ): Extensive resources in patient safety, including primers on patient safety methodology, WebM&M cases on patient safety events, and a regularly updated collection of patient safety journal articles.
- VA Center for Patient Safety: Multiple resources including an orientation to Healthcare Failure Mode and Effects Analysis (HFMEA), a method of trying to determine and correct the ways a system could failure before events occur as well as information on the VA Chief Resident in Quality & Safety program, which provides additional training and experience in patient safety and quality improvement.

- UW Leaf: A user-friendly tool for querying the electronic health system records within UW Medicine in real time. Can also be utilized to query identifiable data with IRB approval. Access requires AMC login credentials and training.
- UW Access to Excellence: Dashboard for visualizing current health system performance in quality and safety. Information can be broken down by UW Medicine clinical entity, service line, and various measurement bundles. Metrics can also be reviewed through equity lenses including race, language, and housing status. Access requires AMC login credentials.
Get Funding
- HQSC Project Mini-Grants: We provide small ($250-$1500) grants to support new or ongoing trainee-led patient safety and quality improvement projects at UW.
- UW Patient Safety Innovations Program (PSIP): Provides pilot funding and scholarship coaching to teams pursuing innovative projects that improve patient care quality and safety, strengthen the patient-provider relationship, and bolster patient safety education.
