If you attended an orientation for this facility in the past and have expired access, please email clatr@uw.edu directly. This form is NOT for renewals and will only delay our ability to assist you with this issue.

Once a training request is received and individual is verified that they are on an approved protocol, an email will be sent out with a list of days and times available for the training. Once a day and time has been scheduled, you will receive a confirmation email with the additional required information to review prior to the training.

Availability

Orientation classes are generally conducted Monday-Friday, between 9:00am and 5:00pm.

Animal Use Medical Screening (AUMS)

You can check your record using the online Animal Use Medical Screening (AUMS) form maintained by EH&S. The AUMS must be complete and documented before you can attend a facility orientation. AUMS must be renewed every three years, or more often as determined by an Occupational Health Nurse.

Annual DCM Facility Access Online Training (Rodent or Non-Rodent)

This course must have been completed within the last 12 months.   It must be completed on an annual basis to retain access to DCM facilities.

Main Campus Facilities

Foege to ARCF Move

Are your animals' housing being relocated from Foege to ARCF?

We recommend that you attend an abbreviated 30 minute orientation that points out the unique aspects of ARCF facility, but you can choose a full 90 minute orientation.

South Lake Union Campus Facilities

South Lake Union buildings requires the additional card that can be acquired through SLU campus services.  If you have not yet received the SLU campus card, enter "N/A".

Harborview Campus Facilities

Research & Training (R&T) is the one animal facility at Harborview.

Harborview requires the additional card that can be acquired through UW Medicine. If you have not yet received the Harborview campus card, enter "N/A".

Other Facilities

ABSL-1 Prerequisite

You must complete the online Animal Facility Orientation Introduction courses before attending a facility orientation.
You can look up your completion date on UW My Research and Training Transcript.

ABSL-2 Prerequisite

You must complete the online Animal Facility Online Learning Course with ABSL-2 before attending a facility orientation.
You can look up your completion date on UW My Research and Training Transcript.

In additional to the prerequisites above for ABSL-3,  before an orientation can occur for this facility there are other things that need to occur.

Finish this form and you will be contacted by the BSL-3 team about those.

This lets the CLATR team know that you need a combined ABSL-1 and ABSL-2 orientation, rather than just an ABSL-2.
