ED-2-ICU: Improving Transitions

Patient outcomes worsen with increasing time in the ED, particularly for critical care patients who require many interventions and high-acuity care. ED/Critical Care fellow Dr. Clint Orloski and attending Dr. Nick Johnson decided to take on these important transitions.

Their team has worked to promote change through culture and systems by revising the Harborview MICU admission policy to promote a “pull” culture, seeking to rapidly triage critical patients and bring them up to the ICU as fast as possible. They have also created additional pagers to promote simultaneous notification of residents, fellows, and attendings when new admissions come in and created a dedicated ED-to-ICU handoff checklist to ensure data does not fall through the cracks during this vital period. The first rounds of data analysis are ongoing, but we are excited to see the results of this exciting work.
