We will be using ASHP’s Pharmacy Online Residency Centralized Application Service (PhORCAS) for all applicants. You can find out more about PhORCAS at www.ashp.org/phorcas.
Our deadline for applications is January 2, 2025 for all programs.
The residency committee reviews your letter of intent, curriculum vitae (CV), transcripts, and three letters of reference.
Please upload your letter of intent through the PhORCAS system. Your letter of intent should:
- State your career goals, why you are considering our program, interests, and what skills/characteristics you would bring to UW Medicine.
- Be specific as to why you feel UW Medicine would be a good fit with your career goals and interests.
- Be no longer than one page.
References should use the standard form in PhORCAS. We do not require a separate free form letter. However, it is important that your letter writers comment on as many of the assessment areas as they are able to.
Please note: University of Washington is NOT able to sponsor visas for pharmacy residents (PGY1 or PGY2).
Program-Specific Application Information
PGY1 Pharmacy
Your CV should include the following:

- Completed and anticipated APPEs, including duration (e.g. 4 week, 6 week, or 8 week rotations). For completed APPEs, highlight unique/advanced responsibilities and integration in patient care activities.
- Pharmacy work experience. Please be sure to include total number of hours worked for each job/internship while in pharmacy school, in addition to dates employed. As with APPEs, highlight unique/advanced responsibilities.
- Leadership and extracurricular involvement. Please note what, with whom, and how often/for how long.
- Teaching and mentorship experience. Please note who, what, when, and time commitment.
- Projects and research experience. Please note your title, role, dates, and time commitment.
Transcripts should be submitted as early as possible. We do NOT require Fall grades or non-pharmacy school transcripts.
At least one of the three required references/letters of recommendation should be from a clinical preceptor.
Please make sure you are designating the correct program. The UW Medicine PGY1 program consists of the University of Washington Medical Center (UWMC) Montlake and Northwest campuses, Harborview Medical Center (HMC), and Fred Hutchinson Cancer Center. The PGY1 residency offered by Valley Medical Center is a separate residency program affiliated with UW Medicine.

Candidates that match with the UW Medicine Health-System Pharmacy Administration residency will be accepted to their respective master’s program. Applicants should not apply separately to the graduate school for the MHA or MHIHIM program.
