Exiting Resident Checklist

By April 29th

  • NAME FOR RESIDENCY CERTIFICATE: FORM LINK Tell us how you want your name to appear on your certificate.

By May 13th

  • GRADUATION PRESENTER: FORM LINK Invite a faculty member to present on your behalf as part of the Graduation Ceremony. Typically, faculty share for around two minutes any remarks they feel will be meaningful for the occasion and conclude by presenting the graduation certificate. Presenters can come in person but if unavailable can pre-record their speech. After your initial invitation or ask, we will contact presenters directly with details. The ceremony is scheduled for Friday, June 21st, 11:30 AM to 5:00 PM. It will be broadcast live and recorded for later viewing via zoom. Venue for after graduation- TBD. Details on End of the Year Banquet and post-graduation celebrations to follow.

By May 31st

  • PHOTO(S) FOR GRADUATION CEREMONY: FORM LINK Upload 1-5 photos of yourself, with or without family, friends, pets, etc. – anything meaningful to you and your residency journey.
  • JOB PLANS: FORM LINK Tell us your plans after graduation including the job title, institution or practice, and location. If this is not known by the deadline, use the Notes section of the form to tell us the current status of your job plans.

Prior to Exit

  • FORWARDING INFORMATION: FORM LINK Provide a forwarding address and confirm contact info.
  • SCHOLARLY ACTIVITY: FORM LINK Complete the ACGME Scholarly Activity form, even if you have no scholarly activity to report.
  • EXIT SURVEY: Complete the anonymous program exit survey which will be sent in June.
  • AAP BENEFITS: Update the information in your AAP account for continued access to benefits such as Peds in Review and PREP: The Curriculum through December 31.
  • SUMMATIVE EVALUATION: Sign and return your Summative Evaluation upon receipt.
  • PSLF FORM: If participating in the Public Service Loan Forgiveness (PSLF) program, submit a completed PSLF form. Forms will be submitted after your end date to ensure credit for the full period of residency.
  • ROTATION SYNOPSES: Submit any outstanding synopses for Advocacy, Away, CAM, Reading, Research and Teaching Electives to Theresa.vu2@seattlechildrens.org. These cannot be credited with out a completed synopsis on file.

Upon Exit

  • ID BADGES: If leaving Seattle Children’s and UW, return your SCH, UWMC and HMC badges to the GME office. If staying, keep your IDs unless directed otherwise by your new program or division.
  • PAGER: If leaving Seattle Children’s, return your pager to the GME office.  Be sure your name is on it. If staying, email us to acknowledge that you have your pager, in working condition.  We will transfer ownership to your new program/division.
  • HUSKY CARD ACCOUNT: If leaving UW, close your Husky card account and request refund of account balance.  If you are owed meal reimbursements for shifts at UWMC or HMC during your final rotation, wait until these are credited before closing the account.  You may keep the card.
  • MAILBOX AND LOCKER: Empty your mailbox and locker in the resident lounge.
  • WHITE COAT: If leaving Seattle Children’s, remove the embroidered logo before disposing of the coat.  (In other settings, white coats have been used in attempts to impersonate medical staff.)
  • NOTES AND DICTATIONS: Complete within 48 hours of your end date. (UWMC/HMC clinical access will be shut off after this time.)
  • MEDHUB: Access to MedHub is available for 30 days after your end date.  Residents moving directly to another UW GME program will lose access to the pediatric residency items in MedHub at 9pm the day prior to the start of their new program.
    • Evaluations: Complete any outstanding evaluations.
    • Procedure/Case Log: Update your procedure/case log in MedHub and export all data to save in your own records.
    • Log Duty Hours: Log duty hours for final shifts.  If unable to do so, contact Becca (robbins@seattlechildrens.org) for assistance right away.

48 Hours After End Date

  • UW EMAIL FORWARDING: When your UW Medicine clinical access ends, you will be able to forward your @uw.edu email to any email address you choose.

2 Weeks After End Date

  • WORKDAY: Update information in Workday at least two weeks AFTER your last day of employment.  Changing your address prior to that date can lead to out-of-state tax withholding on your final paycheck.

Within 30 Days of End Date

  • NPI REGISTRY: Update your information in NPI system (NPPES) within 30 days.

FAQs and Resources for Exiting Residents

DEA Registration

The DEA registration fee is $888 for three years.  Physicians performing certain duties are eligible for fee exemption.  See the DEA Registration Instructions for more details.

Medical License

You should begin applying for a full medical in the state where you plan to practice after residency as soon as this is known.  Application requirements and processing time vary significantly by state.

See the attached Medical License Application Instructions if you have not yet applied for your full medical license.

Professional Liability Insurance

You are insured directly by the University of Washington.  Contact UW Risk Services with any questions.

Medicare and Medicaid Number

You are enrolled in Washington State Medicaid and Medicare with your NPI number.  You can check for any additional Medicare ID numbers through your NPPES account.

Hospital Affiliations/Privileges

When entering this information in credentialing documents, do NOT include hospitals where you worked as medical student, resident or fellow.  This only applies to hospitals where you have been a member of the medical staff.

American Board of Pediatrics (ABP) Certifying Exam

Board dates for 2024 are OCTOBER 15-17,2024.

Application

You should apply for the Boards before your full medical license has been issued Regular board exam registration deadline is April 1, 2024. There is a late registration Deadline and a late fee ($345) if you haven’t already registered by April 1, 2024 until 8:00PM EST May 20. 2024. The final date to apply for the 2023 ABP General Pediatrics Certifying Examination is MAY 20, 2024. No Exceptions

License Requirement

A full, unrestricted medical license is required to sit for the exam.  You must send a copy of the medical license to the ABP by October 3, 2024.

Benefits

Insurance Benefits – Continuation of Coverage

Contact the UW Benefits Office if you wish to arrange continuation of your insurance benefits through COBRA. You will continue to receive your current insurance through the end of the month during which your appointment ends. (e.g. If your last day is June 30th, that will be final date of coverage.)

Retirement Plan

When you end employment at the UW, you keep the accumulations in your UW Retirement Plan—including the UW matching funds. You can contact your record keeper directly if you wish to roll your savings into another retirement plan or Individual Retirement Account (IRA). However, you are not required to make any changes to your account and will be able to access your savings upon retirement or earlier per the policy plan.

Immunization Records

UWMC Employee Health

UWMC Employee Health has primary responsibility for maintaining your immunization records.  To request a copy of your records, email emhealth@uw.edu.  Include your UW Employee ID if known.

Seattle Children’s Occupational Health Safety and Well-being (OHSW)

TB screenings or flu vaccinations given at Seattle Children’s may not be reflected in your UW immunization record.  To request records, email Workforce Health and Safety whs@seattlechildrens.org.  Include your badge number if available.

UW Email

All UW alumni and former employees keep their UW email address indefinitely; however, the service hosting the email inbox expires upon graduation/separation from the university. You will be able to forward this account to a personal email address after your final day of residency.

SCH Email

Access remains active for 30 days after your final day of residency. If access is terminated early, call the help desk, 206-987-1111. Please manually forward or export any important SCH emails prior to July 28, 2024. You will not be able to access emails after the 30 day window and forwarding is not available after cutoff date.
